What Is It?
The Verification of Death form (SMR010.530) is the NSW Health form used to document the clinical assessment that establishes that a person has died.
The NSW Health policy describes verification of death as the clinical assessment process used to establish that a person has died. A medical practitioner normally conducts the assessment. Where a medical practitioner is not available, a registered nurse, registered midwife or qualified paramedic may verify death where permitted by the relevant NSW procedures.
For an expected death at home, the clinical assessment process still needs to be completed.
Who Completes It?
The form may be completed by a medical practitioner, registered nurse, registered midwife or qualified paramedic where permitted under the NSW Health policy and applicable procedures.
For registered nurses and registered midwives, the Statewide Verification of Death form is SMR010.530. The original is provided to the funeral director and a copy is retained in the health care record.
What Happens Next?
Verification of death is not the same thing as certifying the cause of death.
A medical practitioner must complete the Medical Certificate of Cause of Death when the cause of death can be established. NSW Health states this must be completed within 48 hours of death. If the cause cannot be established, or the death is reportable to the Coroner, the matter must be referred to the Coroner instead.
