Form 3 - Permission to Cremate
What Is It?
A Form 3 — Permission to Cremate is the official authority that allows a cremation to proceed in Queensland when the deceased is in the care of the Coroner. It is issued by a Coroner or authorised delegate after the required coronial examination or autopsy.
Note for families: If you have engaged a funeral service provider or undertaker, you do not need to complete or organise this yourself. Your undertaker will arrange the required application as part of the cremation approval process.
Why Is It Important?
The Form 3 provides the Coroner’s permission for the deceased to be cremated.
The Coroner also confirms that the human remains do not pose a cremation risk. This includes considering implanted devices such as pacemakers or other devices that could pose a risk during cremation.
Who Issues It?
The Coroner or an authorised delegate.
What Happens Next?
Once the Form 3 — Permission to Cremate has been completed, the cremation can be booked with a crematorium.
